今天是我上医学系第二年第一次第一天的考试
第二年和第一年真的差很多，就在科的方面就比较专了
要学很多，好像量血压、心肺复苏、看 X-RAY、心电图、看诊、听诊、听心跳声、洗手、穿手套等等等。
都很具有挑战性。
今天我就是考这些实习上的东西，还有包括看心脏标本认血管= =
考试早上10:15分，我七点半就起床，一边读书心脏一边抽筋，
太紧张了，心脏不是在跳动，而是发酸 （你明白我的意思吗？）
之后就排队进入考试的站点。
这次有分很多站，一个站只有5分钟的时间作答，之后铃声响起，就到下一个站。
考场里每一个站点还用医院用的屏障遮起来，看起来就像真的很专业的考试一样，也和电影里医学生们考试的场景一样。
我竟然爬到医学系第二年了，我竟然就坐在这个考试站里，
十几年前我做梦也想不到我会成功到达这个等级，
虽然以后还是有好长的路要走，好多的试等着我去考。
这一次真正亲自做的是问诊，大学请了一个马来男中学生来让我们考试。
我一进去，就看到那名马来人躺在那里。
我就把昨天记好的问诊顺序很顺利的做了一次。
以下是
Good morning. I am Lim. I am going to examine u. Are you agree?
马来人点头说好。
我在病人的床前站住，观察他
The patient does not have respiratory distress（呼吸困难）, and the nutritional status is good.
然后走到他的右边，
What is today?
Friday.
What did you have just now as breakfast?
Sandwich.
The mental status is good and the patient is conscious.
may I have a look for your hand?
他点头说好
The palm is dry and warm, looks pink, that is normal.
No Osler's node and Janeway lesion （这些是心脏病的症状）
The nail bed is pink, no peripheral cyanosis, no splincter hemorrhage, no finger clubbing .
There is no tendon xanthomata.
There is no needle puncture marks on the arm.
然后就量脉搏。
The radial pulse is 60 beats per minute, rhythm is regular, volume is normal, and the caliber of the vessel is normal and elastic.
Can you please turn you head to the left side?
我就量颈动脉。
The carotid pulse can be felt and there is no raise in jugular venous pressure.
我把他的眼皮往上拉，can you please look down?
The sclera is white and do not have jaundice（眼白是白色的，没有黄疸病的征兆）
接着拉下下眼皮，can you look up?
The palpebral conjuctiva is pink, there is no anemia.
Can you open mouth?
Dental hygiene is good and do not have nicotine stain.
Roll up your tongue please.
No central cyanosis.

我把手放在他胸前，心脏的部位。
I am going to feel your apical impulse.
The apex beat is normal, confined to 1 intercostal space.
I am going to calculate your respiratory rate （呼吸次数）
然后老师就叫我跳过，因为要算一分钟。
接着我去检测他的双脚有没有水肿(pitting pedal edema）
There is no pitting pedal edema.
完了，我就跟他说谢谢。
（其实考完后我才想起忘了做一样是，就是叫他坐起来检查他的颈项淋巴，看有没有肿）
之后老师就问我，如果jugular venous pressure上升，你要怎么量？
旁边放着两把尺。
我把短的那把放在他的sternal angle 上，then another longer ruler placed on the top of the raised jugular venous pressure. Two rulers are perpendicular to each other.
老师就点头说好，之后还称赞我 very good，称赞了两次！！（心花怒放）
之后又再去到下一个站，继续我的考试，直到结束
哇！林康宁！
你又过了一关了！
之后和朋友们谈天才发现原来我问诊的那个病人还算不错，其他的问他什么，好像现在是什么时候？回答 NO。早上吃了什么？一个回答NO，一个回答吃了 plate（盘子） = =
超爆笑的！
弄到我的朋友不知道要宣布他是神志清醒还是神志不清。
不过我的那个也蛮好笑，在我前面的宛仟问他早餐吃了什么，他说tosai（一种酸酸的印度煎饼）。
我就问宛仟，我跟你的是同一个人厚？
他跟我说他吃了三明治。
然后全部都在爆笑 。
太可爱了
总之，还是谢谢那个马来人，很听话的让我顺利考完，不向其他的那么顽皮 XPP
